To ensure the upcoming asthma presentation is directly relevant to your needs, please complete this survey.

To be eligible for 5 CPD points per hour, each GP is required to:
(1) complete THIS questionnaire and return it to____________(local division)_______ by __(4-6 weeks before event)_____;
(2) complete a workshop evaluation form (provided at the workshop); and
(3) complete a post workshop questionnaire 4 weeks after the presentation.

GPs who do NOT complete all three forms will only be eligible for 2 CPD points per hour when they attend the presentation.
