GP Service Coordination Workshop, March 2002

GPDV convened a statewide workshop on GP service coordination on 27th March. 25 PCPs and 21 divisions attended. This is the first of 3 workshops to be held in 2002 bringing together divisions and PCP staff with the following goals:

GP Service Coordination Workshop, July 2002

GPDV convened a second statewide workshop on GP service coordination on 31 July, 2002. Participants included: 18 division staff and 25 PCP staff/agency reps, as well as representation from DHS and state office of DoHA.

This is the second workshop to be held in 2002 bringing together divisions and PCP staff with the following goals:

  • To illustrate the work currently being undertaken between divisions and PCPs to achieve quality care planning and effective referral between GPs and other service providers as a basis for service coordination
  • To provide an overview of current issues, challenges and response options for division and PCP staff to consider in their joint work
  • To enable participants to identify successful strategic approaches to their joint work.
