Medication Management
APAC Guidelines
In 2002 the Australian Pharmaceutical Advisory Committee (APAC) developed Guidelines for Medication Management in Residential Aged Care Facilities (external PDF KB)
Resource Kit
In 2006 in Victoria the Department of Human Services developed a resource kit for implementing the APAC guidelines. The kit contains practical ideas to assist with implementation and sample procedural forms for committee meetings and draft policy documents.
Resident Medication Management Review
The RMMR is the Aged Care Facility equivalent of a Home Medication Review. From July 1 2007, new arrangements for pharmacists involved with aged care facilities were implemented. Pharmacists now undertake one RMMR per resident each year. Also, a GP is able to initiate an RMMR for a resident. Further details are available from the Pharmacy Guild and from Medicare.
