Cold Chain Breach Reporting

 

For all cold chain breaches please contact DHS on 1300 882 008.

 

Please have all relevant information collected, i.e. Clinic details, how long the vaccines were out of range for, record of all vaccines being held and in what quantity, any possible explanation for the breach having occurred, e.g. power failure.
