
Myhealthepractice�
Myhealthepractice� refers to a range of GPV projects which aim to support Victorian divisions to build the IM capacity of general practice to achieve improvements in the quality and use of general practice data. Projects within the program currently support many of the goals of the Australian Better Health Initiative (ABHI), Australian Primary Care Collaborative (APCC), population health and chronic disease management and e-Health activities.
The first two projects under the Myhealthepractice� banner is to support divisions to implement Pen Computer Systems (PCS) Clinical Audit and Adelaide West General Practice Network (AWGPN) Practice Health Atlas.
Clinical Audit
The PCS Clinical Audit System (CAS) is a clinical information system which continuously supports quality improvement in information management and enhances the business capability of general practice. This package works with the GP Clinical Desktop System (Medical Director 2 and 3) to present the GP and practice staff with understandable clinical information to which they can respond.
There are increasing, and sometimes unwelcome, demands on general practice to participate in the collation and dissemination of de-identified patient information to support population health initiatives. Clinical Audit will assist practice staff to assess the quality and completeness of patient information, and therefore, practice population information, while identifying the target patient groups that, appropriately managed, can increase income from practice incentive payments.
GPV will conduct a study on identifying the socio-technical barriers and opportunities which support the use of Clinical Audit by GPs & general practice. Practice Health Atlas
Practice Health Atlas (PHA), developed by Adelaide West General Practice Network, is a decision support tool that aims to inspire general practice teams to reflect on their activities and to develop innovative business models for more effective health care services and outcomes. It is based on the synthesis of relevant, high quality and timely practice health data, as well as using such data to predict future health care needs and trends (intelligence).
