Examples of Questionnaires to Practices
To better meet the needs of practice nurses in their areas, many divisions have sent questionnaires out to all practice nurses (or practice managers) to gain a greater understanding of nurses' profiles, current skill levels, current duties, needs etc.

Systems Thinking Audit Tool
A checklist developed for use by division staff when visiting practices. Based on the outcomes of the GPV Chronic Disease & IMIT workshop (24 April 2002) and on the "systems thinking" approach developed by Monash University. It allows divisions/practices to audit their practice systems that support the employment of a practice nurse.

PEN Clinical Audit Systems�
The PCS Clinical Audit System (CAS) is a clinical information system that continuously supports quality improvement in information management and enhances the business capability of general practice. This package works with the GP Clinical Desktop System (Medical Director 2 and 3) to present the GP and practice staff with understandable clinical information to which they can respond. There are increasing, and sometimes unwelcome, demands on general practice to participate in the collation and dissemination of de-identified patient information to support population health initiatives.
What Clinical Audit will do is assist practice staff to assess the quality and completeness of patient information, and therefore, practice population information, while identifying the target patient groups that, appropriately managed, can increase income from practice incentive payments.
UPDATE: testing of clinical audit with Best Practice and Genie is currently underway.

Practice Health Atlas and Practice Mapping�
Practice Health Atlas (PHA), developed by Adelaide West General Practice Network, is a decision support tool that aims to inspire general practice teams to reflect on their activities and to develop innovative business models for more effective health care services and outcomes. It is based on the synthesis of relevant, high quality and timely practice health data, as well as using such data to predict future health care needs and trends (intelligence). Back To Top
