Primary Care Co-ordination

 

Overview

 

To provide co-ordinated primary health care in Victoria GPs and other primary health care services need to work together effectively. In an environment where there are multiple service providers in both the public and private sector this is quite a challenge. GPV works with Victorian divisions, the State Department of Human Services(DHS), allied health peak organisations and disease specific organisations to facilitate co-operative working relationships between all primary care service providers.

 

GPV's role is to:

    1. Inform and support divisions in their efforts at the local level to develop systems in general practices and partnerships that make it easier for practices to work with other service providers
    2. Inform other relevant primary care services and organisations of initiatives affecting general practice
    3. Advocate to other organisations about what would make it easier for general practice and divisions to work co-operatively with other primary care providers and organisations.
