GP Liaison Workshop - 18th March 2008

 

Objectives

 

  • To update participants on HealthSmart developments
  • To provide participants with information to assist them in reporting on their annual plans
  • To assist participants to share key lessons for effective discharge from the acute to community setting

 


Update of the EARC committees/ Clinical Networks

  

Margaret Grigg, Manager Statewide Emergency Program gave a brief presentation on the DHS clinical network initiative and provided feedback on the EARC subcommittees that include GP representatives.

 

 

Update on HealthSmart

 

Antonio Abbenante Manager, Integration Services for Clinicals, OHIS provided an update on the HealthSmart program including the work on the discharge component and the role of the HSD.

 

 

Information re the PCP Strengthening Partnerships, sustainable collaborations, building resilient communities workshops

 

Adele Hamlyn, Primary Health Branch, Senior Project Manager, Partnerships Team provided information on the content of training being offered through primary care branch on change management and partnership development

   


Reporting on General Practice Liaison

 

Lenora Lippmann, Integration Team Leader, GPV introduced this session by briefly outlining the benefits of reporting in health programs. Annual Plans: Jane Measday, Consultant GPV briefly outlined the reporting process and GPV reporting to DHS.

 

 

Discharge from the acute to primary care setting

 

Using the 7 building blocks of discharge as the framework, GPL units shared their experiences as a starting point for open discussion on tips of success for improving discharge from the acute to community.

  

  • Building Block 1 Patient nominate a GP for continuing care

       Alfred GPL �Linking Patients presenting at ED with GPs in the community�

       RCH GPL �All Kids need a GP� Campaign and Kids Connect �Finding a GP� website

  • Building Block 2 GP name on patient file

  • Building Block 4 Designated person at ward level responsible for discharge

  • Building Block 5 Hospital to have mechanism for notifying GPs of an admission

       Austin GPL Unit

  • Building Block 6 Hospital staff to see job includes safe referral home

       St Vincent GPL Unit

  • Building Block 7 Medical Staff to see value of the GP role

       Peninsula Health GPL Unit

 


Google Maps for Divisions

 

Brendon Wickham, eHealth Support Officer GPV demonstrated the application �Google map for Divisions� and discussed the applicability for health services and �find a GP campaign�
