Hospital Admission Risk Program
The Victorian Government established the Hospital Admission Risk Program (HARP) in 2001-02 as part of the Hospital Demand Management Strategy. HARP was designed to reduce the rate of growth in demand for acute services by �collaboratively developing preventive models of care between acute and community providers, targeting people with manifest health needs who are frequent users of the hospital system�. The emphasis was on better supporting and proactively managing people in their homes and the community instead of reactively responding to acute exacerbations of their conditions.
Key features are:
- co-ordination between services and across sectors
- targeting patients that will benefit from additional services.
HARP involves a collaborative effort by hospitals, community services and general practitioners in each stage of a patient�s care.
