Collaboratives - 'Beyond the Collaborative'

The workshop aims to reengage and enthuse practices who have been involved in Phase One of the Collaborative. Participants will be updated about changes in the topic areas.

Hear from fellow practices about how they have sought to improve their own care systems, sustained change in their practices, progressed Quality Improvement work into other topic areas and met patients needs in a Population Health framework.

There will also be opportunity to gain an understanding of the potential of the PEN Clinical Audit Tool and the Practice Health Atlas.
