The agenda and faxback registration form for an event is faxed or emailed out to all Victorian and Tasmanian Divisions of General Practice approximately four weeks prior to the event and contains the agenda for the day, venue information and the due date for registrations. Registrations usually close about one week prior to an event. To secure a place as a participant at a GPDV event, please ensure you register by the appropriate date. Please note:
Registrations need to be made in writing on a registration
form. If you have not received the faxback registration
form for an event, please contact Eliza at GPDV (contact details
provided above) or download and complete the generic registration
form above. Once completed, send your form to Eliza by either fax
or email before the closing date for enrolment (approximately one
week before the event).
GPDV offers Orientation to Division Workshops for Program and Administrative Staff. These workshops are scheduled on demand. If you are interested in attending an orientation workshop please fill out the appropriate expression of interest form below or email Eliza at e.sanneman@gpdv.com.au. You will then be notified of the next orientation workshop.
