Statewide Referral Form - Revised
The new up-to-date Victorian Statewide Referral Form (VSRF) is now accessible by clicking on the link below. Please note, that the template is in 'rtf' format and requires to be saved as an 'rtf' file only for it to work in Medical Director. To do this please follow instructions below. In addition we have included links to download two examples of the same form in 'word' format - one copy with blank fields and the second with sample data included.
Instructions: Upon downloading file, please hold cursor over the link, right click on mouse and click on 'Save Target As...'. This will allow you to save the template as an 'rtf' file. The file name extension will be listed within the bottom tab area of the 'Save As' window.

VSRF Instructions
This is a brief guide to the use of the Victorian Statewide Referral Form (version 4) as implemented in Medical Director. The purpose of this template to provide a standardised mechanism for referral from General Practice to State funded Health Providers. The format and content is somewhat different to a routine General Practitioner’s referral letter but with minimal effort transfer of relevant, information is easily produced. General Practitioners are encouraged to use this template as a replacement for the multitude of service specific referral forms.
