Broad Aims

This workshop will showcase activities and projects that enable general practice to participate in coordinated approaches to health promotion and service coordination in primary care. In particular, participants will have an opportunity to discuss strategies and opportunities for achieving the following outcomes:

• Enhance GP referral to and feedback from primary care agencies
• Implement referral pathways and other service arrangements that support better chronic disease management, and
• Identify how chronic disease prevention can be strengthened through coordinated division and PCP/agency activity
