GPDV CDM Reports to DoHA

Implementation Plan Analysis
In December 2002, GPDV submitted its first report to the Department of Health and Ageing (DoHA) for the Chronic Disease Management (CDM) contract.  This report, for the first nine months of the contract, includes a synopsis of GPDV activity.  In addition, DoHA asked each state-based organisation to undertake a statewide analysis of Divisions’ CDM Implementation Plans.

First Interim Report - Analysis of Divisions Business Reports (Jan - Jun 02)
In February 2003, GPDV submitted its first interim report to DoHA for the CDM contract.  This is an analysis of the first six monthly Business Report of Divisions for January - June 2002.

Second Interim Report - Analysis of Divisions' Business Reports (Jul - Dec 02)
This is the Second Interim Report to the Department of Health and Ageing (DoHA) for the Chronic Disease Management Initiatives.

The components provided in this report are:

  • Section One - An analysis of the Victorian divisions' business report on the Chronic Disease Management (CDM) Program contract, for the July to December 2002 reporting period.
  • Section Two - A synopsis of GPDV activities at a state level across program areas including, mental health, practice nurses and IT/IM. GPDV also has provided a summary of Outcomes achieved in Victoria and Reflections of GPDV progress to date.
  • Section Three - A summary of barriers to the uptake of the items.

  • Section Four - A comprehensive package of Chronic Disease Statistics.

Fourth Interim Report - Analysis of Divisions' Business Reports (Jul - Dec 03)
This is the 4th SBO Interim Report for the period 1st July 03 to 31st Dec 03. We undertook this analysis by reading all of the division' reports for this period and plotted key activities and approaches using the analysis tool that we developed with the Centre for Integration Studies at the University of NSW, which has been used throughout this program.
