The HMR is a consumer-focused, structured and collaborative health care service provided in the community setting, to optimise quality use of medicines and consumer understanding of their medicines. It involves the consumer, their general practitioner, and their pharmacy.  The program involves a home visit by an accredited pharmacist, who reports back to the patient's GP.  The patient and the GP then formulate a consumer-focused medication management plan.
