Transferring a new resident to an aged care residential facility from the acute care setting: a regional medication management service
Lisa Spurling is a pharmacist, who has worked in the southern region of Adelaide both in the hospital and community settings for the last 5 years. The research presented are interim results of a single blind randomised controlled study of older adults facing first time transfer from acute care to the residential care setting within the southern region of Adelaide. They aimed to determine whether a Regional Medication Service could improve health outcomes for this high risk, frail group and to determine medication-related issues and barriers along the continuum of care between hospital and residential care facilities.
