Allied Health Item Number

From 1 July 2004, Medicare will cover services provided by some allied health professionals and dentists as part of a GP care plan for treatment of a complex condition.

  • Patients with chronic medical and complex care needs whose care is being managed by an EPC CDM multidisciplinary care plan are eligible for up to five allied health services and three dental care visits per calendar year on referral from the GP.
  • GPs are required to provide GP Management Plan Service GPMP Item 721 & undertake Team Care Arrangements Item 723 or contribute to the Care Plan Item 731 of a resident in an Aged Care Facility prior to referral to the Allied Health and Dental Care Services. GPs that have existing EPC care plan in place may also refer eligible patients to these services.
  • Reference to how the provision of the Allied Health Service will assist the patient acheive goals are to be itemised as part of the Care Plans provided to patients.
