Paper No. 1 Audit of Current Practice	The aim of the audit was to document current practice on admission and discharge notification and policies that were seen by hospitals and divisions to be effective. The audit also informed the development of the database of GP contact information.
Paper No. 2 Minimum Requirements for the Transfer of Information between hospitals and GPs	The purpose of this paper is to establish a set of minimum requirements for the transfer of admission and discharge notification between hospitals and GPs. The requirements were initially developed after consultation with Divisions and hospitals throughout Victoria. The consultation formed part of the statewide audit of current practice that included 31 Divisions and 36 hospitals.
GP Register Specification	The General Practitioner Register (GPR) is an initiative of the Department of Human Services and Commonwealth Department of Health and Aged Care, implemented by General Practice Divisions of Victoria to provide a centralised repository of accurate information about general practitioners and the contact details for the locations where they work. This is in response to a growing demand for accurate GP information for automated notifications and electronic discharge summaries.
