Victorian Workcover Certificate MD template for the recently change Victorian Workcover Certificate. The actual statutory certificate is a numbered preprinted form and the output from this template is printed on to the form in the applicable places. Please note: this template will require some modifications for each installation, due to the different line feed characteristics on printers. This is outlined in the instructions. |

| Divisions Standard Data Items Database | Developed by Gary Holzer and Andrea Deussen from the South
Australian Divisions of General Practice Inc. The database was designed to
assist rural divisions to manage the reporting and
evaluation components of their program. It is equally
useful to metropolitan divisions. Note: Microsoft Access 2000 is required to run the database. Conditions of use: The database can only be used by divisions of general practice and SBOs and is not be redistributed or used for commercial purposes. |
