Small Grants for Integrated Health Promotion in General Practice
The Primary Health Branch of the Department of Human Services has a limited amount of one off funds available in 2005/2006 for small scale projects to support the health promotion/illness prevention role of General Practitioners, through the Primary Care Partnerships (PCP) platform. The focus of activity to be funded by the grants program will be strategies that better integrate General Practice and Community Health Services.
General Practice Division(s) in partnership with PCP(s) are invited to apply for non-recurrent funding of up to $20,000 to build on work already underway through most Divisions of Geneal Practice in relation to implementation of the Lifestyle Scripts initiative (part of the focus on Prevention package in the 2003-04 Federal Budget).
