Objectives

This workshop has been designed to explore features of general practice capacity, which may contribute to providing quality chronic disease management.  Participants will have the opportunity to:

• Understand the elements of practice capacity, which are being investigated as contributors to providing quality chronic disease care in general practice.
• Explore in detail IM maturity and teamwork as key features and consider the implications for the division’s approach.
• Receive a CDM program update.
