Program Planning for GP Panels - 8 October 2004

Objectives:

  • Explore how needs assessments have been used as a strategy for engaging aged care homes
  • Have the opportunity to network with neighbouring divisions and discuss models for 'GP Panels'
  • Learn about Re-AIM and program logic theory as tools for program planning and evaluation
  • Receive an update from the Department of Health & Ageing as to the roll out of the GP Panels initiative
  • Increase knowledge about the MBS Items in Aged Care to more effectively utilise data in program planning and evaluation

Session 1: Needs assessments as a means for engagement

Discussion regarding the use of needs assessment as a strategy for engaging aged care homes. Presentations will be a starting point for facilitated discussion about local strategies used for engagement.

Session 2: Program Planning & Evaluation

The aim of this session is to assist delegates to use the RE-AIM framework and a program logic approach when planning, implementing and evaluating programs. This presentation will provide an overview of program logic theory and tools to take away and practice back at the division.

Session 3: Update from DoHA & MBS Items available in Aged Care Homes

Update from DoHA on the Commonwealth survey of aged care homes which is the key data source for the program & the MBS item statistics soon to be available to divisions.

Session 4: GP Panels: emerging models

Participants will have the opportunity to share where they are up to in planning for their GP panel, what panels are starting to look like, challenges and opportunities so far, payment models, to discuss the key issues emerging and look at opportunities for collaboration.
